Substance Use Disorder and Co-Occurring MH/SUD Case Manager

 

The Substance Use Disorder and Co-Occurring MH/SUD Case Manager plays a vital role in our behavioral health team by supporting patients with substance use disorders (SUD) or co-occurring mental health and substance use disorders. This position involves assessing patient needs, coordinating services across medical, behavioral, and community resources, and advocating for patients to promote recovery, stability, and improved quality of life. The Case Manager works collaboratively within a multidisciplinary team in an outpatient FQHC setting to deliver patient-centered, trauma-informed care.

Key Responsibilities

  • Implement and monitor individualized service plans in collaboration with patients, behavioral health providers, primary care clinicians, and other team members.
  • Coordinate referrals and linkages to internal services (e.g., psychiatry, therapy, medication-assisted treatment) and external resources (e.g., housing assistance, SUD treatment programs, vocational support, peer recovery services, social services).
  • Provide ongoing case management support, including crisis intervention, follow-up, and advocacy to ensure continuity of care.
  • Facilitate patient navigation through complex healthcare and social service systems, addressing barriers to access.
  • Monitor patient progress and document services accurately in the electronic health record (EHR) in compliance with FQHC standards, HIPAA, and state/federal regulations.
  • Participate in interdisciplinary team meetings, case consultations, and quality improvement initiatives.
  • Educate patients and families on and available agency and community resources.
  • Support warm handoffs between primary care and behavioral health services.
  • Maintain knowledge of community resources, evidence-based practices, and best practices in integrated care for mental health and SUD.
  • Tracking utilization of case management services, as well as patient successes.

Qualifications

Required:

  • High School Diploma
  • At least 2 years of experience in case management, behavioral health, mental health, or substance use disorder services.
  • Knowledge of mental health conditions, substance use disorders, recovery models, and trauma-informed care.
  • Strong understanding of social determinants of health and experience working with diverse, underserved populations.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work collaboratively in a team-based environment.
  • Proficiency in documentation and EHR systems.

Preferred:

  • Experience in an FQHC, integrated primary care, or community health setting.
  • Certification in case management or behavioral health case management.

Physical Requirements

  • Ability to perform office-based work, including computer use and telephone communication.
