In general, SCHC uses your health information for the purpose of treatment, payment and overall healthcare operations. In accordance with the established 1996 HIPPA Law (Health Insurance Portability and Accountability Act), we will not disclose your protected health information to others or for other purposes unless specifically directed by you, the patient, or unless the law authorizes or requires us to do so.

• If we refer you to another provider
• When ordering lab work
• Appointment reminders
• To collect payment for our services
• To contact you to provide information about our services

• The right to opt out of communications for marketing
• The right to be notified in the event your information was breached
• The right to receive a copy of your full SCHC medical record
• The right to inspect and request and amendment to your SCHC medical record
• The right to request a restriction on the health information we share with others
• The right to request who we communicate with about your health care and the method in which you choose communication

If you believe your privacy rights have been violated, you may file a grievance with the Sapphire Community Health HIPAA Privacy Officer or with the Secretary of the Department of Health and Human Services. All complaints must be submitted in writing. See 45 C.F.R. § 164.524.
