Factors That Affect Your Health

At Sapphire Community Health Center, our Patient Navigator helps patients connect to outside resources they need in order to focus on getting and staying healthy. This process involves evaluating and addressing the Social Determinants of Health (SDoH) in each patient’s life.

Social Determinants of Health are conditions in which people work, live and age. They may include such things as access to housing, transportation, food, social support systems, personal safety and financial stability. Healthcare professionals now recognize the impact these or the lack of these may have on a person’s overall health.
